Health Benefits

Eligibility: Regular employees who work a minimum of 20 hours per week.

Coverage effective date: Coverage is effective the first of the month following 30 days of employment.

Description: EBMS of Montana provides a traditional 80/20 plan with a Flexible Spending Account/Dependent Care option or a High Deductible Health Plan with a Health Savings Account. Both plans include Pharmacy coverage.

Paid: Benefit is paid by RiverStone Health for full-time employees. Monthly premium is pro-rated for part-time employees.

Eligibility: Regular employees who work a minimum of 20 hours per week.

Coverage effective date: Coverage is effective the first of the month following 30 days of employment.

Description: Dental coverage is included within the EBMS of Montana medical plan.

Eligibility: Regular employees who work a minimum of 20 hours per week.

Coverage effective date: Coverage is effective the first of the month following 30 days of employment.

Description: Prescription drug coverage is included within the EBMS of Montana medical plan.

Eligibility: Regular employees who work a minimum of 20 hours per week.

Coverage effective date: Coverage is effective the first of the month following 30 days of employment.

Description: Two plans are available: Davis Vision and VSP.

Paid: Benefit is paid by employee.

Eligibility: Regular employees who work a minimum of 20 hours per week.

Coverage effective date: Coverage is effective the first of the month following 30 days of employment.

Description: Offered through Unum Life Insurance Company, at 75 percent of annual gross salary (capped at $40,000).

Paid: Benefit is paid by RiverStone Health.

Eligibility: Regular employees who work a minimum of 20 hours per week.

Coverage effective date: Coverage is effective the first of the month following 30 days of employment.

Description: Offered through Unum Life Insurance Company.

Paid: Benefit is paid by RiverStone Health.

Supplemental Benefits

Eligibility: Regular employees who work a minimum of 20 hours per week.

Coverage effective date: Coverage is effective the first of the month following 30 days of employment.

Description: Offered through Unum Life Insurance Company; employee pays at various rates.

Retirement Benefits

Eligibility: Mandatory contribution of 7.9 percent for employees working more than 960 hours per year. Contribution is optional for employees working less than 960 hours per year.

Coverage effective date: Immediate upon hire

Description: Plan is administered by the Montana Public Employees’ Retirement Administration (MPERA).

Eligibility: All employees are eligible

Coverage Effective Date: Open enrollment

Description: Two options: ICMA-RC Services or Nationwide Retirement Solutions

Time Off Benefits

Eligibility: All employees

Effective Date: Eligible to use accrued vacation hours after six months from date of hire; eligible to use accrued sick hours three months from date of hire; holidays are paid based on work schedule from date of hire.

Description: Employees proportionately accrue hours based on the number of hours worked in a year.
