RiverStone Health Home Care Services has been serving the Yellowstone community for more than 40 years.  We are the first provider of home care services in Montana to be nationally accredited by the Community Health Accreditation Program (CHAP).  Created in 1965, CHAP recognized the need and value for accreditation in community-based care and is the oldest national, community-based accrediting body with more than 5,000 agencies currently accredited nationwide.  CHAP accreditation assures the highest standards and quality of care is provided to our home care patients.

RiverStone Health Home Care Services may be requested by physicians, the patient or family member, or a hospital discharge planner by calling 406.651.6500 during business hours.  Through consultation with the patient’s healthcare provider, a plan of care is developed to meet the patient’s individual healthcare needs.

Cost of services may be covered by Medicare, Medicaid, private insurance or private pay.  A sliding fee scale is available for those who qualify.

Our team includes nurses, physical therapists, occupational therapists, speech therapists, medical social workers and certified home health aides.
